WestCare Transitional Counselor in San Luis Obispo, California
Person in this position will be responsible for developing transition plans for inmates who have been assessed and have received treatment services, and are being transferred into a community based treatment setting.
Essential Job Functions:
Provide information and guidance to participants ;
Provide information on services provided at Day Reporting Centers, Parolee Service Centers, outpatient and residential treatment programs, and any additional transitional services developed by CDCR or the Counties.
Assist participants in completing the appropriate forms regarding the transitional care phase of the program services;
Work in cooperation with the other contract staff, CDCR parole staff, the continuing care service providers, county probation, and the participant for the development of the Community Service Plan and Discharge Plan;
Shall record in participant's file, participant 's attendance in continuing care presentations, and meetings with parole, CCIII and/or the PSA to discuss plans;
Shall work collaboratively with the CDCR and institutional staff;
Attend all required staff development training, including CDC's Cross-Training, and WestCare in-service training such as conducting assessments, clinical skills, developing service plans, emotional incarceration , relapse prevention , motivation for treatment and stages of change, cognitive skills, conflict resolution, social skills, moral development, communicable diseases, and self-help groups;
Embrace and embody the mission, vision, guiding principles , clinical vision and goals of WestCare Foundation; and
Perform any other duties as assigned.
Knowledge of offender populations, criminal subcultures, and cultural differences, substance abuse assessment tools, therapeutic community concepts, familiarity with operations of Day Reporting Centers, Parolee Service Centers, Residential Multi-Service Centers, Specialized Treatment for Optimized Programing (STOP), Female Offender Treatment and Employment Programs, outpatient and residential treatment programs, and any additional transitional services developed by CDCR or the Counties;
At the time of hire, must be certified as an AOD Counselor with a certification recognized by DHCS;
Must have the ability to obtain and maintain CDCR clearance;
A valid California driver’s license and ability to obtain and maintain WestCare vehicle insurance; and
CPR Certification, First Aid Certification and an Annual Tuberculosis test.
Education and Experience:
Associates Degree and two (2) years of experience providing cognitive behavioral treatment to the offender population, or
Four (4) years of experience as a certified counselor providing transitional services.
- Work is primarily performed in an in-prison treatment setting.
The normal work routine involves no exposure to blood, body fluids, or tissues, BUT exposure or potential exposure may be required as a condition of employment. Appropriate personal protective equipment will be readily available to every employee engaged in Category II tasks;
Occasional lifting of more than 10 pounds;
Transportation of clients in company vehicle;
Occasional risk of intensive physical activity with clients; and
Sitting, standing, walking, reaching are performed in the normal course of the position.
